PART 1: Apply to St. Marcus School using Skyward registration. CLICK HERE.
PART 2: Apply for an MPCP voucher through the Department of Public Instruction (DPI). CLICK HERE. The application is available the 1st-20th of each month.
PART 3: Submit these required documents to St. Marcus via email at email@example.com:
Current immunization records required. Email a copy (firstname.lastname@example.org) or have your doctor's office fax a copy.
